What MI Choice may cover
MI Choice is designed to help people who meet nursing-facility-level care and financial eligibility requirements receive approved long-term services in a community setting. Depending on the person’s authorized service plan, support may include:
- Assistance with bathing, dressing, grooming, toileting, and mobility
- Medication administration and care coordination
- Safety supervision and transfer assistance
- Case management and individual service planning
- Support that helps maintain independence and daily routines
Who determines eligibility?
Michigan Medicaid and the assigned MI Choice waiver agency determine eligibility, the authorized services, and the reimbursement level. Park Place does not approve Medicaid benefits and cannot guarantee coverage or timing.
A practical application path
- Contact a local waiver agency. Ask to begin the MI Choice screening and application process.
- Complete financial and clinical reviews. The agency will explain required records and conduct assessments.
- Discuss placement with Park Place. We evaluate whether an available apartment and our care model can meet the person’s needs.
- Coordinate the service plan. If approved and admitted, Park Place works with the supports coordinator on authorized services.
