Levels of Care

Standard Services

Three (3) daily meals, administration of medications, assistance with bathing, weekly housekeeping, personal laundry, cable television, and group activities.

Level I: Offered in Cedar, Dogwood or Evergreen Place

Level I residents receive standard services. The resident must be alert and able to move throughout the facility independently.

Level II: Offered in Cedar, Dogwood or Evergreen Place

Level II residents receive standard services plus one or more of the following on a daily basis: Help with dressing, grooming, toileting and/or bathing; assistance from staff with walking, wheelchair escort, or transferring.

Level III: Offered in Aspen and Birch Place

Level III residents receive all of the services offered in Levels I & II plus one or more of the following services: Assistance with feeding, special / pureed diets as medically ordered, assistance from two (2) staff persons with activities of daily living including: bathing, dressing, toileting, ambulating and transfers, assistance with incontinence, security of a locked and fenced building, and specialized medical services.

Level IV: Offered in Aspen Place ONLY

All the services of Level I, II & III are offered to the Level IV residents within the security of our Specialty (Memory Loss) Unit plus one or more of the following services: Assistance from staff for behavioral support, specialized activity program, and individualized activities.

Hospice Care

Hospice services are warmly welcomed into our homes. Hospice allows us to care for the terminally ill residents in the comfort of their own rooms surrounded by their personal belongings and loved ones.

Respite Care

Respite and short-term stays are also welcome. The stay can be as little as a few days or for even months at a time for recuperation or to give family and caregivers a break and some much needed R&R. Whatever the reason, you are welcome at Park Place.
